Assumption of Risk
Basketball is a physical activity involving risks including:
- Falls
- Contact injuries
- Sprains
- Fractures
- Concussions
- Heat-related illness
- Other unforeseen injuries
Parents acknowledge these risks.
Medical Authorization
Parents authorize:
- Emergency medical treatment
- Emergency transportation when necessary
- Communication with emergency contacts
Parents remain responsible for medical expenses.
Insurance
Participants are responsible for maintaining personal medical insurance.
Future 360 League does not provide medical insurance coverage for participants.
Code of Conduct Agreement
Parents agree to:
- Encourage sportsmanship
- Respect officials
- Respect coaches
- Respect opposing teams
- Avoid abusive language
- Follow facility rules
Players agree to:
- Compete fairly
- Respect teammates
- Respect opponents
- Respect coaches
- Respect officials
- Display positive character
Release of Liability
To the fullest extent permitted by law, parents release Future 360 League, Shoot 360 Cordova, its officers, employees, volunteers, sponsors, and affiliates from claims arising from participation, except where prohibited by law.
Electronic Signature
Parents must electronically acknowledge:

- Medical authorization
- Emergency contact verification
